Athletes are REQUIRED to have all forms turned in BEFORE trying out for a sports team including Parent/Athlete Code of Conducts and Travel Release Form. Please email all completed forms to athletics@cornerstoneclassical.org.

**Additionally, athletics fees MUST be paid before athletes receive a uniform. Unpaid athletic fees will cause the athlete to become ineligible to practice/play.**
